New patient InformationPlanning for your first appoint - here's how: Your first visit will last approximately 30-60 minutes. If you had x-rays taken at your dentist’s office, we may be able to use them. If not, it is likely we will need to take any necessary to aid in your care. We prefer periapical type films generally, although others may be needed depending on the condition. During your first visit we will discuss with you your health history and your periodontal condition. We will complete an examination and discuss any findings. An additional appointment may be needed to develop a thorough care plan. Your dentist will be notified as to what is needed once a care plan has been agreed upon; and a detailed letter will be sent to your dentist outlining our findings and the recommended care needed. What to bring to your first appointment The medical history form filled out We want your time with us to go smoothly. So please understand that if you have had a dental examination within the last 6 months, your insurance company will not cover your first visit with us. We ask that payment be made for this visit at the time of the visit. It will be submitted to your insurance company and if it is paid, you will be reimbursed. Cancellation Policy If an appointment needs to be cancelled or rescheduled, we ask that you give us 24 hours notice. |
